Randomized Clinical Trial of Intermittent Calorie Restriction in People with Multiple Sclerosis: Effects on Immunometabolic and Cognitive Measures

Abstract BACKGROUND: Intermittent calorie restriction (iCR) reduces inflammation in preclinical models and people with Multiple Sclerosis (pwMS). Mechanisms could be related to changes levels of adipose tissue-derived adipokines reduced systemic gut microbiota composition. AIMS: Evaluate the effects iCR on levels, metabolic immune/inflammatory biomarkers compared a control unrestricted diet (Ctr) pwMS. METHODS: PwMS (relapsing Remitting MS) were randomly assigned or Ctr groups for 12 weeks. Blood was collected at baseline, 6 Leptin adiponectin peripheral blood immunologic profiling, anthropometric total body fat measures, clinical cognitive measures evaluated. Differences between examined using linear, repeated mixed model adjusted baseline age, sex DMTs. RESULTS: 42 pwMS randomized (iCR=22 Ctr=20). 34 completed study (iCR=17 Ctr=17). showed significant reduction (weight BMI) adiposity (waist circumference mass) over decreased significantly lower group weeks approaching significance Adiponectin increased from The decrease Th1 cells an increase CD45RO+ regulatory T after Cognitive testing symbol digit modality test greater improvement versus CONCLUSIONS: Short term is safe feasible pwMS, it can ameliorate metabolic, immunologic, profiles.
